
If you would like to receive our Home Visiting service, you will be provided with a monthly visit from one of our experienced Care Coordinators.
What do the Care Coordinators do?
The First 4 Support Care Coordinators have put hundreds of service users in touch with invaluable support services across the district, and can help with a wide variety of requests from obtaining advice about claiming welfare benefits, to helping to arrange handy persons and gardening services.
During their first visit, they will complete a thorough assessment of your needs which involves setting up a Support Plan to identify any additional help that you may wish to access, in addition to offering their own recommendations as to what services are available that we think could prove beneficial.
The Care Coordinator will then provide continued support during their monthly visits, which includes:
• Re-assessment of your needs every 6 months
• Helping to maximise your welfare and benefit entitlement
• Speaking to health and social care agencies on your behalf to set up any care and support services you need
Is there anything they can't help with?
Although our friendly, professional Care Coordinators will endeavour to provide the support you need, they are not able to perform the following tasks:
• Personal care, including bathing and dressing
• Preparation of food
• Collection of prescriptions or administering medication
• Shopping
• Being witness to any legal documents
